Burial Fund
The Pokagon Band’s Burial Fund Code established a program that provides financial assistance with funeral and related expenses at the time of death for those that are covered by the Burial Fund. The Burial Fund provides a benefit amount of $10,000, of which up to $8,000 is provided for the funeral, $1,500 is provided for a monument, and $500 is provided for a funeral luncheon. Any remaining funds are paid to the Named Beneficiary, which can be more than one person. If the decedent failed to provide the Band with a Named Beneficiary, then the remaining funds are paid to the decedent’s surviving spouse. If the decedent did not have a surviving spouse, the remaining funds are paid to the decedent’s estate.
If eligible as defined under subsection 6(B) of the Burial Fund Code please download the Designation of Beneficiary Form. After completing the Designation of Beneficiary Form, please return it to the Department of Social Services at
